Legal Notice & Participant Responsibility Acknowledgment

By registering for and/or attending any program, event, class, or activity offered by First Five Club, whether held at our facility or off-site, you acknowledge and agree to the following:

Assumption of Risk: Participation in First Five Club programs may involve physical activity and interaction with other participants and/or equipment. By enrolling, you voluntarily assume all risks associated with participation, including but not limited to minor injuries, illness, or other unforeseen incidents.
Parental/Guardian Responsibility: For programs involving children, parents and guardians are solely responsible for the supervision, behavior, and well-being of their children, unless otherwise specified in writing by First Five Club staff.
Health & Safety: You agree not to attend any program if you or your child are experiencing symptoms of illness or have been recently exposed to a contagious condition. First Five Club reserves the right to deny participation to anyone exhibiting symptoms that could compromise the health and safety of others.
Waiver of Liability: By participating, you release and hold harmless First Five Club, its owners, employees, contractors, and partners from any and all claims, liabilities, or damages arising from your or your child’s participation in any program/activity/class/events, etc., including those hosted by First Five Club at external locations.
Photo/Video Release: Unless otherwise requested in writing, you grant permission for First Five Club to capture and use photos and videos taken during programming for marketing and educational purposes.

By proceeding with registration, you confirm that you have read, understood, and agree to this notice in full.
