Please register for this activity to complete the required Health History Form for Summer Camp 2026.

PLEASE NOTE: Non-Members are only eligible to register for S.T.E.A.M Camps and S.T.E.A.M Camps with Montclair Learning Center. All Traditional Camps, Leadership Camps, and Sports Camps require a YMCA membership. 

It is strongly recommended that you complete this in advance of registration day as it will expedite your registration for camp. If you have multiple children that will be participating in camp, a seperate form is required for each child.  

Please have the following information ready prior to filling out the form:

• Contact information for emergency contacts
• Contact information for the child's doctor and dentist
• Child's vaccination history
• List of medications your child takes
• Health insurance information 

Price: $0.00 Taxes waived

Required age: 3 - 15 years old on the day of the activity

Notes: This form is required for each participant who is participating in ANY camp run through our YMCA.
