This is the form for all entities, recurring, and/or commercial-gain users. It is applied for at any point in the year, and expires at the end of the calendar/fiscal year.  

It is a prerequisite for reservation of any space in our system by non-personal entities, those generating commercial gain 

In 2026 the approved application fee is waived; in future years it will be $50.

If your application is approved, you will be emailed a link to upload your Certificate of Insurance and pay the approved application fee.
