PHOTOGRAPHIC RELEASE: I understand that photographs may be taken during this activity and hereby grant the District permission to use any such photo(s) for advertising or in promotional materials.

HEALTH & SAFETY

The registrant and participating enrollee(s) shall comply with all applicable public health rules, regulations, orders, and/or guidance in effect at the time of the class/activity, but not limited to, physical distancing, facial coverings, use of appropriate sanitation practices, etc.

The District or Contracted Instructor reserves the right to remove or prevent class/activity participation if the registrant or enrollee fails to comply with any provisions of the health and safety measures.

Should you need to transfer to another class or request a refund due to COVID-19, please complete a Refund Transfer Request.

REFUND POLICY

Full refund will be given if the Fair Oaks Recreation & Park District cancels the program or for medical reasons.
No refund will be issued after the class or program has had its second meeting.
All refund requests will be approved or denied by the Fair Oaks Recreation & Park District. All refund checks will be processed through the County of Sacramento and mailed to you within 4-6 weeks. Credit card refunds will show on your bank statment in 24-48 hours.
The Fair Oaks Recreation & Park District reserves the right to adjust the policy, depending upon the circumstances and the severity of the situation.
On-line convenience fee is non refundable.
All participants requesting refunds are required to submit a Refund Transfer Request.
Policy does not apply to the following areas:
