Summit Membership

Download the Annual Summit Membership PDF. The Waiver & Release of Liability, Annual Membership Agreement, Consent for the Release of Information To and From, and Marketing & Promotional Photo Consent Form must be signed by the participating member and/or a Legal Guardian annually.  

Description:

  • In order to participate in recreational programs, you must be a member of The Summit
  • Membership is valid for 1 year from the time of purchase
  • Membership prices are as follows:
    • Individual - $40/Year
       
  • New participant's membership will be reviewed by The Summit Program Manager
  • New Members may be asked to attend a program trial session before registering for a program
  • Information provided on your membership form will help inform Summit Staff on how to best support you in programs.
  • Personal Health Numbers (PHN), Medication Administration Records (MAR), and other medical details will be utilized in case of emergencies (e.g., information may be shared with medical professionals).

If the individual is 19 and over and has a Representation Agreement or Committee, his/her Representative must sign.

You can submit this by... 

  • Uploading a scanned (or photo) copy to your Smart Rec account 
  • Emailing summitprograms@idsbc.org 
  • Delivering in-person to Summit staff  

Note:

Summit reserves the right to revoke memberships, ask individuals to attend with a Personal Support Worker, or refuse program enrolment. This is the case in the following situations: 

  • Requires assistance with personal care routines 
  • Needs assistance with physical mobility 
  • Needs assistance to stay with the group  
  • Has a Behaviour Protocol, or has behaviours that may reduce the quality of service to the public; place themselves, the public, or the team in danger. 
  • Epilepsy or Seizure Protocol
  • Needs assistance with medication administration
