I acknowledge that there is potential risk for injury involved in training and competing in any sport. By submitting and signing this form, I acknowledge that I am aware that there are risks associated with gymnastics. I warrant that the participant named on this information form, is physically fit to participate in gymnastics. I declare that I have accurately disclosed all information regarding physical, mental or medical conditions affecting the named participant and acknowledge that this information may be used for the Club/Gymnastics Ontario’s (G.O.) use in the delivery of a gymnastic program. I understand that the Sault Ste. Marie Gymnastics Club has tried create a safe and controlled environment for participation and that the Club has established rules for participation on and about the gymnastic area that must be followed by the participant. I understand that failure to comply with any of the policies and rules of the Club and/or Gymnastics Ontario may result in the suspension or termination of membership. I waive the rights of the participant to damages or other costs in the event injury is caused due to participation in gymnastics.

Recreational Class Cancelation Policy

If for any reason you are unsatisified after your child's first class, we will provide you with a full refund. If you wish to cancel after the second week of classes, we offer a pro-rated refund. After the third week of classes, no refunds will be granted. There will be a $25.00 administrative fee for cancellations.

Note: The Sault Ste. Marie Gymnastics Club reserves the right to cancel class offerings that are not full. Parents/Guardians will be notified to discuss alternate class arrangements.

Inclement Weather Policy and Unavoidable Cancellations (Power Outage)

If local schools are closed due to weather, daytime programs will automatically be cancelled. Should after school and weekend programs be cancelled our Website and Social Media accounts will be updated immediately to notify the membership.

Should classes be cancelled due to a power outage, we will immediately update the Website and Social media account.

***There are no refunds or makeup classes for cancellations to the above situations***

GENERAL GYM RULES
Safety first at gym

Think “safety first” in gymnastics
Coaching supervision is always required in the gym
Coach permission is required to:
- enter gym
- go on equipment
- try new skills
- perform aerial inversions or flips
- leave the gym
Understand the risk of the activity
Be prepared to participate:
- be healthy and not tired
- wear proper clothing and footwear
- have long hair tied back
- always warm up
Stop unsafe and careless acts which could cause injury by listening
- following instructions
- controlling your actions
Participate cooperatively within the limits set by the coach
Double check equipment
Master basic skills before attempting more advanced skill
Keep your body under control
Follow equipment and safety procedures
Be alert and safety conscious
- watch where you are going
- walk don’t run
- watch for obstacles or uneven surfaces
Respect others’ performances - no distractions
Parents and spectators must stay in their designated area

Allergies
Our facility is peanut-free. Our allergy list changes based upon the registered members for each session. Please ensure you have advised of allergies at registration or notify the office immediately.
