I have full knowledge of the risks involved in participating in the program and am voluntarily assuming the risks involved;

Notice of Risk: When children from multiple families attend a day camp, there is an increased risk of the COVID-19 virus coming into the camp. Children who are infected with the COVID-19 virus are more likely than adults to have very mild infections or to have no symptoms at all, but these children can still transmit the infection to other children and to adults at the camp. This means that children can bring home an infection acquired in a camp and put other persons at risk. Even if a day camp has a screening process to help detect infections when there are symptoms; this screening process will not detect infected children or adults who do not have symptoms at the time of screening.

I fully understand that I will be solely responsible for any injury, loss, or damage caused or sustained;

I release the Manor Park Community Council (MPCC), the City of Ottawa, and their respective employees, volunteers, or instructors from any claims, damages, actions or causes of actions arising out of or in consequence of any loss, injury, or damage to any person or property incurred;

I authorize the MPCC to collect and use personal information collected on this form for the purpose of management and administration of programs.

I authorize the MPCC to disclose such personal information to third parties, including legal and insurance companies, for the purpose of management and administration of programs.
