En contrepartie de l'utilisation des installations de LA CORPORATION DE LA VILLE DE HAWKESBURY, je, pour moi-même, mes héritiers, exécuteurs, administrateurs et mandataires renonce à tout recours dont je pourrai bénéficier lors de toute blessure dommage, perte et libère LA CORPORATION DE LA VILLE DE HAWKESBURY, ses administrateurs et actionnaires et tout autre représentant, employé, mandataire ou toute autre personne qui assiste aux activités et les dégage et libère de toute responsabilité pour tout accident ou dommage souffert directement ou indirectement par moi-même à l’occasion ou à la suite de ma participation aux activités chez LA CORPORATION DE LA VILLE DE HAWKESBURY Par la présente, je déclare que ma santé physique me permet de participer aux activités du LA CORPORATION DE LA VILLE DE HAWKESBURY et être conscient des risques inhérents des activités du LA CORPORATION DE LA VILLE DE HAWKESBURY.

LA CORPORATION DE LA VILLE DE HAWKESBURY se réserve le droit d’annuler sans avis ou appel, une accréditation accordé à un membre pour une activité effectuer sur ses lieux ou pour une activité organiser au nom ou par LA CORPORATION DE LA VILLE DE HAWKESBURY.

Release and Waiver
In consideration of the use of the facilities of the CORPORATION OF THE TOWN OF HAWKESBURY, I, for myself, my heirs, executors, administrators and assigns, waive any claims to which I may become entitled for injury or damage and release THE CORPORATION OF THE TOWN OF HAWKESBURY, its directors, officers and shareholders and all other representatives, employees, agents or any other person assisting in the activities of THE CORPORATION OF THE TOWN OF HAWKESBURY from any claims for damages, injury or loss suffered by me as a result of my participation in the activities of THE CORPORATION OF THE TOWN OF HAWKESBURY. I further state that I am in proper physical condition to participate in the activities of THE CORPORATION OF THE TOWN OF HAWKESBURY and am aware that participation in such activities could in some circumstances, result in physical injury.
THE CORPORATION OF THE TOWN OF HAWKESBURY reserves the right to revoke without notice or appeal any accreditation that it has issued for any activity to be carried out on its premises, or for any activity carried out on behalf of, or by THE CORPORATION OF THE TOWN OF HAWKESBURY.
