For the purpose of this agreement, any participant or parent listed in the preceding registration process shall be considered a registered person.

I authorize the Leaside Baseball Association ("LBA") (including its officials, coaches, umpires, staff, and volunteers) in the event of an emergency to use its reasonable discretion, on behalf of me, the registered person(s) or any associated spectator(s) participating in or attending at one or more of LBA’s current or future programs, to render first aid treatment and/or arrange for emergency medical care (including hospitalization), at my expense.

I understand that LBA will take every reasonable step to ensure the health and safety of the participants and spectators, however, I also understand that the programming involves demanding physical sport in which injuries might reasonably occur to participants or spectators. I acknowledge that notification, in writing, of any changes to the medical condition of each registered person(s) must be received by LBA prior to the start of the registered person(s) LBA programming.

I recognize that playing or attending the game of baseball or related programming can contain serious risks. I release and indemnify the LBA and its directors, officers, shareholders, partners, staff, employees, consultants, volunteers, representatives, predecessors, successors and assigns from any and all claims and liabilities whatsoever arising from the participation in or attendance at one or more of LBAs current or future programs by me, the registered person(s) or any associated spectator(s).

I acknowledge that participation in or observation of the LBA’s programs involves a voluntary assumption of all the risks associated with the games and activities being played and that the LBA cannot remove risks to participants and spectators. I understand that the LBA is in no way affiliated, associated or in any other way connected or related to the Toronto District School Board, Toronto Parks, Forestry and Recreation or the City of Toronto (the “Unaffiliated Parties”); I, therefore, release and indemnify the Unaffiliated Parties from any and all claims and liabilities whatsoever arising from participation in or attendance at one or more of LBA’s current or future programs by me, the registered person(s) or any associated spectator(s).

I further grant to the LBA, its representatives and employees the right to take photographs and/or video of me and/or the registered participant(s) and/or associated spectator(s) in connection with the LBA. I authorize the LBA, its assigns, and transferees, to copyright, use and publish the same in print and/or electronically.

I agree that the LBA may use such photographs or video of me and/or the registered participant(s) and/or associated spectator(s) with or without our name(s) and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content. I further authorize the LBA to provide any collected non-medical registrant information to the Ontario Baseball Association Inc. ("Baseball Ontario") and consent to Baseball Ontario, or its partners, to contact me about this registration, commercial offers and/or other information.

I further agree to abide by the rules, regulations, policies, and bylaws of the Leaside Baseball Association and consent to the use of any and all private information of myself or the registered person(s) herein provided in accordance with the Personal Information Protection and Electronic Documents Act in this year or any successive years of participation.

Under Rowan's Law, before any player can be registered with the LBA and Baseball Ontario, the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review one of the Concussion Education Resources on an annual basis provided by the Province of Ontario and must review the Player Code of Conduct.

I hereby confirm that the player registered to participate in baseball activities with the LBA and Baseball Ontario, and the parent or legal guardian of the player being registered if the player is under 18 years of age, have reviewed one of the concussion education resources provided by the Province of Ontario as referenced above and have reviewed the player code of conduct as referenced above. I/we further acknowledge understanding the nature and risk of concussion and head injury to athletes, including the risks of continuing to play after a concussion or head injury is suspected.

I assume full responsibility for payment and acknowledge that I have read, understand and agree to all of the LBA's policies as outlined herein. I acknowledge and agree that the LBA can rely on all representations, acknowledgments, and agreements made and information given by me. I acknowledge that I am also acting as agent of any other parent/guardian of the registered person(s) and with authority to enroll the registered person(s) in LBA programming and to execute this agreement on their behalf and that the LBA may fully rely on my authority in connection with all such matters.

I understand and agree that the provisions of this agreement are severable, separate and distinct and the unenforceability in whole or in part of any provision hereof shall be deemed not to affect or impair the validity or enforceability of any other provision hereof.

In consideration of the LBA accepting the registered person(s) in its programming, I grant the registered person(s) permission to participate in any LBA program(s) and agree to all preceding terms.
