Wellington Parks and Recreation Waivers and Agreements

Liability Waiver

On behalf of myself and any other person on whose behalf these terms are now agreed, I expressly understand and agree that neither the Town of Wellington, Colorado, a municipal corporation, nor any of its officers, agents, volunteers, assistant, or employees shall be held responsible or subjected to any claim seeking to assess damages or establish liability for or arising from personal injury, property damage, or loss of any other sort as a result of actual or proposed participation in the program for which I am now registering. I hereby agree to indemnify and hold the Town of Wellington, Colorado, its officers, agents, volunteers, assistants, or employees harmless on account of any such personal injury, property damage, or other loss as aforesaid.

Permision to Use Photograph

On behalf of myself and any other person on whose behalf these terms are now agreed, I hereby grant full and unrestricted permission to the Town of Wellington Parks and Recreation Department to use photographs in publications of the Town of Wellington Parks and Recreation Department, and to release photographs to media outlets for publicity purposes.

By agreeing to these terms and conditions, I acknowledge that I have read and understand this Liability Waiver and Permission to Use Photographs, and freely and voluntarily agree to this document on the date here after set forth.
