WAIVERS AND PRIVACY POLICY:

1) FREE HOUSE DANCE PLUS LTD (hereinafter referred to as "FHDP") will not assume liability for any lost or stolen property or for any and all liability for any loss, damage, expense or injury (including death). I hereby agree to waive any and all claims that I have or may have in the future against FHDP and its directors, officers, shareholders, agents, employees, representatives, successors and assigns (hereinafter collectively referred to as the "Releasees") and release and indemnify the Releasees from any and against all liability for any loss, damage, expense or injury (including death) that I might suffer or that my next of kin may suffer arising from or in connection with participation in or practicing of any physical training, travel, athletic activity or contest through, with, in any way involving or in association with FHDP or use of or presence on or in premises owned or operated by FHDP or for the time being used by FHDP.

2) Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

WAIVER OF LIABILITY I, would like my child/children/I to participate in dance education classes at Free House Dance Plus (the Dance Studio). In response to efforts to contain the spread of COVID-19 coronavirus, I am aware that I am using the facilities at my own risk and will ensure that I will conduct myself in a way that is appropriate. I confirm that I have not travelled outside of Canada for the past 14 days and have not been in contact with anyone who is suspected of having COVID-19. I do not have any symptoms of COVID-19 including but not limited to fever, sore throat, coughing, aches, respiratory problems, dry cough, fatigue, diarrhea, loss of taste or smell, chest pain, rash on skin, discolouration of fingers or toes, headache. The Dance Studio has put in place preventative measures to reduce the spread of COVID-19; however, the Dance Studio cannot guarantee that you or your child(ren) will not become infected with COVID-19. Attending any public spaces increases your risk and your child(ren)’s risk of contracting COVID-19. By agreeing to this waiver, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Dance Studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at The Dance Studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, The Dance Studio employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the Dance Studio or participation in programs at the Dance Studio. On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless the Dance Studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the Dance Studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Dance Studio program.

3) PRIVACY: FHDP is committed to maintaining the security, confidentiality and privacy of your personal information. We collect, use, disclose, store and retain personal information to the extent required to provide you with dance instruction and related services. We restrict access to your personal information to only those employees and instructors who must use the information to provide services to you. We also endeavour to maintain physical, electronic and procedural safeguards to protect your personal information from unauthorized access, use, alteration or loss. We endeavour to keep accurate, complete and up-to-date personal information in our possession, so if you have updates to your personal information please contact us.
