We, the staff of Feather River Elite recognize our obligation to make our participants and their parents aware of the risks and hazards associated with the activities offered by Feather RIver Elite. Any activity involving height and/or motion creates the possibility of serious injury, paralysis and even death from landing on the neck, head and/or other parts of the body. Mats and pits do not eliminate this hazard. Participants may suffer injuries, possibly minor, serious or catastrophic in nature. With the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child or children participate in the programs offered by Feather River Elite. I fully understand and will direct my child that there are risks and dangers associated with participation in gymnastics events including but not limited to bodily injury, partial and/or total disability and death. These risks may be caused by the negligence of the participant or negligence of others and there may be other risks not known to us at this time. I, my executors or other representatives, waive and release all rights and claims for damages that I or my child may have against Feather River Elite and/or its representatives whether paid or volunteer.

I fully understand that staff members of Feaher River Elite are not physicians or medical practitioners of any kind. With the above in mind, I hereby release the Feather River Elite staff to render temporary first aid to my child/children in the event of any injury or illness. Should the Feather River Elite staff deem necessary or at the request of the parent/guardian, emergency response personnel will be summoned for treatment and/or transportation to the nearest appropriate medical facility.
